For Referring Veterinarians

We accept transfers and referrals from all shareholder and non-shareholder clinics. Please send a transfer sheet with the pet if it is coming from your clinic. To download a copy of the transfer sheet, click on Transfer Sheet for Shareholder Clinics and Transfer Sheet for Non-Shareholder Clinics. Please indicate on the transfer sheet any treatments, diagnostic tests, presumptive diagnosis, special requests and if you want the pet to return to your clinic the next business day.

Please call our clinic to inform the staff that you are transferring a patient. If the veterinarian on duty is not able to talk to you, please leave an evening telephone number for the veterinarian in case there are additional questions.

Please send the following with your client:

·
Transfer sheet including any special requests
·
X-rays, copies of laboratory work or medical records – anything that be helpful
·
Fluids/Medications*
·
Your after hours phone number or contact information
·
Emergency fee information
·
Directions to the St. Louis Animal Emergency Clinic

*Please note that we will accept opened fluids and IV lines from the referring DVM for use with the patient. The Animal Emergency Clinic will not accept syringes of medications, unopened bags of fluids or bottles of medications that the Animal Emergency Clinic carries. If you wish to have the patient on a specific medication which our clinic does not carry, the original bottle of medication (or properly filled out prescription bottle if the medication is oral) must be sent with the client. Any remaining medication will be returned to you via the client.

Please inform your clients that we do not accept payments, post dated checks or care credit.

Once your client arrives at our clinic, we will try to make it a priority to see your patient. If the clinic is busy and the patient is critical, we may ask the owner if we can take the pet directly back for continued care, after which the receptionist on duty will fill out the paperwork and obtain the clients information. Every patient will receive an estimate and will be asked to leave a deposit. Please call us if you have any questions.

Feel free to call the DVM on duty at any time to discuss ongoing care or to address any questions or concerns.

Thank you for your referral.
